Private patient advocacy
Our Services
-
Facilitate communication between patients, multiple care providers and specialists, and family members ensuring that all parties are informed and aligned.
-
Organize and facilitate family meetings in person and virtually to discuss plans of care. We use healthcare ethics frameworks to ensure autonomy is preserved whenever possible, all voices are heard, and we arrive at a solution that works for everyone.
-
Help explain and create your your advance care plans and living wills.
-
Monitor patient’s health conditions and treatment outcomes, providing regular updates and intervening when necessary. Ensure patient’s rights, preferences and care needs are being adhered to and advocating on their behalf if the are not. Monitor facility adherence to treatment plans, safety protocols and regulatory standards, addressing discrepancies if found.
-
Review billing claims and fight denials on your behalf.
-
It is estimated that up to 50% of older adults do not take their medications as prescribed. Working together we can ensure medications are understood and schedules make sense.
-
New diagnoses are overwhelming. We can help research options, discuss second opinions, and even attend appointments to increase recall of new information.
-
Transitions of care are some of the riskiest times in healthcare. Addressing communication gaps can decrease medication errors and confusion during handoffs from one facility to another. We can also provide one on one time after discharge to enhance patient and caregiver understanding of new conditions and complex instructions.
-
We can help navigate if or when to enlist hospice or palliative care services, as well as evaluate options.
